Chapter CEU Process
IARP will submit CEU applications for IARP Chapters for CRCC, CDMS, and CCMC.
Please use up to date application forms and be sure they are signed.

The forms need to be provided to IARP staff by 6 weeks prior to the event. After that, the chapter will have to pay a late fee. Please include your credit card information on the form. Approval of CEUs is not guaranteed and applications submitted late may not be approved by the time of the event.
Please include the following documents:
2 copies each of the applications for CRCC, CDMS, CCMC. These should all be signed.
4 copies each of the following documents: flyer for event, agenda with times, goals or objectives for each session, blank sign in sheet, evaluation form for each session, bio or CV or resume for each speaker.
