This position is responsible for case management for individuals with chronic pain in the Greater Baltimore area.  Activities will include: onsite assessment of individuals with pain management problems, collection of data, establishing relationships with injured workers, families, providers, carriers and employers, authorization of services, development and implementation of care plan in coordination with team members, and documentation and evaluation of injured workers progress.

The ideal candidate will have the following: - RN, Current Maryland unrestricted nursing license; CCM preferred - Clinical or case management experience in managing pain and chronic pain diagnosis. Through knowledge of healthcare delivery systems, standards of care for chronic pain, substance abuse and behavioral health diagnoses, and public and private resources required. Excellent verbal and written communication skills; - Proficiency in Windows based applications with experience in Internet research and Microsoft applications; - Strong organizational skills, proven ability to meet deadlines and ability to prioritize and maintain a high volume workload; Experience working from home preferred; Extensive(almost daily) business related travel is required in the local area.
